Working with Children’s and Transition Teams
Our education and social work specialists can support teams to develop their skills and knowledge base to enable them to work confidently with families.
At the end of the course, staff will be able to:
• Familiarise themselves with relevant and procedures, including local ISP placement procedures regarding post-16 options.
• Consider and present all options available to best meet the stated required outcomes and aspirations of the young person in line with the EHCP and identified transition pathways, including personal budgets, social care and health, local community and education based resources.
• Discuss cost-effectiveness and Value for Money and how this should impact on decision making and share this with families.
• Make outcome focused decisions based on relevant documentation and views of the young person and family.
• Draft/agree a set of guiding principles which will enable frequent honest, communication whilst recognising collective experience, skills and knowledge and encouraging creative thinking and problem-solving.
